Who we are

St. Joseph's Healthcare Hamilton is the organizational sponsor for the Halton Geriatric Mental Health Outreach Program. The outreach program is part of St. Joseph's Geriatric Psychiatry Service and is affiliated with Mc Master University supporting and promoting academic development in a wide range of learners. As part of the broader regionally based service, we are able to tap into the resources and expertise found within our inpatient geriatric psychiatry behavioural assessment and stabilization unit (A1) located at St. Joseph's Healthcare - West 5th site (Hamilton) and St. Joseph's other community-based geriatric psychiatry programs serving Brant, Hamilton and Niagara.

Mandate & Funding

The mandate of the Geriatric Mental Health Outreach Program is to provide and enhance continuity of care for seniors with late onset mental health problems, addiction and/or behavioural disturbance (responsive behaviour) by:

The Halton Geriatric Mental Health Outreach Program was originally developed under an innovative, multi-partner Service Agreement in June 1999. The funding allocations and organizational partnerships have changed and evolved over time. Currently the program operates with partnership funding agreements between St. Joseph's Healthcare Hamilton and Joseph Brant Memorial Hospital, Halton Healthcare Services, Halton ADAPT and more recently, Trillium Health Centre. Funding from Ontario's Aging At Home Strategy through the Mississauga Halton LHIN has significantly increased the program's ability to respond, better serve and support high risk seniors living with complex mental health and/or addiction issues and their families/caregivers.

Target Population

We serve adults 65 years of age and over residing in the regions of Halton and Northwest Mississauga who present with the following:

The age range is flexible for adults with age-related memory impairment (e.g. early onset Alzheimer Disease).

Our Model of Service

The model of service for the Halton Geriatric Mental Health Outreach program can be best described as a flexible approach; striving to understand the experience of seniors and families; sharing, and partnering with primary care, community, LTC and hospital-based services; appreciating the service and system limitations and strengths; optimizing the clinical abilities and resources internally and externally across our service partners and the system; positioning our service as a resource; and demonstrating a commitment to building capacity within ourselves and others

Our team works in a trans-disciplinary manner with training in:

Responsibility for delivering primary health care to the seniors rests with the family physician or alternative primary care provider, community agencies and long-term care facilities. Our consulting physicians work in a shared care fashion and make recommendations to primary care physicians and others. Interventions are conducted in person or by phone with the client, the family, significant others, service providers or even at the system level. Interventions can include health teaching, supportive counseling, psychotherapy, person-centered monitoring and tracking, advocacy for services and/or supports, case management and coordination, family conferencing, client-focused educational sessions for service providers in LTCH, adult day programs, supportive housing, etc.
