How to Make a Referral

The need for geriatric mental health outreach services may be triggered or identified by a wide range of sources to include primary care physicians, specialists, emergency department staff, in-home community support agencies, adult day programs, families and long term care homes and even seniors themselves.

We encourage and request people considering the need for outreach program involvement to speak with the family physician and share your observations/concerns. Referrals to the program must have the approval of the family physician. We are a shared-care model of service and work in partnership with the primary care physician.

For those persons who do not have a family physician please contact Health Care Connect at 1-800-445-1822. Health Care Connect helps Ontarians who are without a family health care provider (family doctor or nurse practitioner) to find one. People without a family health care provider are referred to a family doctor or a nurse practitioner who is accepting new patients in their community. For more information about Health Care Connect:

If you are unsure about a possible referral, please do not hesitate to contact the program office at 905-681-8233 or 1-866-429-7677 and discuss the scenario with our staff.

Referrals to the program are screened for criteria and priority. When making a referral, please provide:

Submission of an incomplete referral form (e.g. missing demographic information, no physician signature etc.) can delay the service delivery process.
